Please Note: BOTH forms must be submitted to Accounts Receivable before FMP can process your request for a credit account with FMP.
We recommend that new customers submit the Customer Set Up form to Accounts Receivable, even if the FMP Credit Application is not submitted. Both forms may be mailed, faxed, or emailed to Accounts Receivable.
